OK, now that I am doing the new activity participation 3.0, I get it. However I have been informed by 2 consultants that we do NOT have to complete a progress not due to the assesments being so "all inclusive" with the information they require. Such as all the narrative writting in inputing all of the info. I am including SO much information that I just don't know what to include on the progress not anyway that is not repetative of what I have written in the assesment. I am still doing the note because I am nervous of this. However if I really don't have to do it I'm ok with that. My consultant said that the info in the assesment is available to the state surveyers if the want to see it and only to write a note if I NEED to include other info not already covered. Has anyone else heard this?? After all it is ultimatly my "butt" on the line not my consultants, however this would be great and save so much work.

Well, let's take solace in the fact, that we are all confused as hell. I find it almost comical, at this point, that so many of my regional peers who I have talked to directly have different interperatations of what the proceedures and protocols are all about. Ya know what, I think the "Creators/Drivers" of the new MDS.3 engine are confused, as well. At the end of the day, they don't know how or are unwilling to answer questions because they just don't know themselves. Honestly, I just wish the process (for activities) be all self contained in the MDS. I just can't wrap myself around the redundancy of a 2 step documentation proceedure (same info goes in the chart as well as inputing into MDS ) Obviously, I think it is going to take 2 months or so before all the kinks are banged out. To hope for a consistent operating standard might just be a tall order. I think each facility, ultimately is going to figure out their own "design" system. Raps(CAA's whatever you want to call them) in the past, at my facility were inputed as quite simply "see careplan or progress notes". So my point is if we have to check off and write a narrative into the MDS, then that should suffice and some how be included/copied/pulled electronically into the individual Resident's medical records chart. Does this make any sense, or do I really not "get it". Obviously, I know I have not been to or seen web/seminars. I am so dissapointed in my admin/corporate leadership for not being more proactive in providing guidance!

I'm with ya lo !!! I feel exactly the same. I'm just going to do what I do & hope for the best. I guess "it is what it is"...frustrating, exhausting & most of all time consuming to say the least. For now I'm going to NOT do the progress note & I'll see what happens. ALL of the activity info is on the MDS Assmt. sooooo.....I don't know. They should have had all this figured out prior to implementing it!!!!

If I were you I would at least write a note that states "Initial assessment completed." That way you're at least covered.

I mentioned that to our DON and she has not heard that. We have had much inservice training over the past 3 months on the MDS 3.0 and none of the trainers has mentioned that. I'm not going to say they told you wrong because I am totally clueless about this but boy how I hope they told you right! It would be a blessing to not have to do that much documentation anymore!

We are going to still do 1/4 progress notes hand written to add to the chart just so we can lshow that we have more to add to the Residents care than the " preferences" . My goodness I know we are more valuable than just that!
