I just have to toot my own horn. We had full survey last week. I was watched all week but what was amazing was they NEVER asked for even 1 piece of documentation!This has been my 6th year as an activity director and they have always asked for calendars, resident counsel minutes, 1:1 documentation. So I was ready!! After the survey I asked one of the state reps why I was not asked to show any of this documentation. She told me that there was no need because they could tell by many signs that the activity department was absolutly great! She said the atmosphere of the building was #1, the residents were extremely happy with my program, the 1:1s were being done where needed, my groups were well attended and there were no residents just "Parked". I asked her if we should keep documentation and she said absolutely because you may need it at anytime in the future. Also my facility is a 106 bed SNF in Texas and I am the only person in my department... no assistant just myself and volunteers. |

congrats, Patti~ this is extremely positive! Of course, she does make the point that future surveyors may want documentation, so you're stuck with still doing it. But perhaps more work with surveyors and governing bodies to streamline the documentation would be a win-win. And obviously, your activities program speaks for itself when the surveyors do not need paperwork because they see evidence all around! |

We had our full annual survey in January (I am in Michigan). When the surveyors came in, same as every year (for the past ten years) they ask to see the past three months activity calendars and my resident council minutes. They hold a special Resident Council Meeting (residents and surveyor only) and all went well. They also did not see anyone just "parked", loved my activity program, could see by the calendars that I do not do the same thing every day, every week, every month, and in the council meeting, resident stated that they love me, enjoy the way that I interact with them by providing a variety of activities so they don't get bored. They stated "You never know what she is going to do"! The only complaints they had were nursing and dietary. They never asked to see my 1:1 program or documentation or my progress notes, that is not to say they did not look at my progress notes becasue they are in the residents medical charts. Activities did not receive any citations, and have not received any in the ten years I have been here! Whoo-Hoo! The surveyors have in the past asked to look at my participation logs and my 1:1 documentation so it depends on the surveyors and what they see or read in the charts. I continue to document because I feel that no one can remember what happened for each resident month to month or for the year and when a surveyor wants information, "If it is not documented", they truly believe "It didn't happen", because you have no proof! Here's a scenario: Let's say, based on the overall good "vibe" surveyors get from interviewing/observing the Residents within the context of activities, they still insist on "crosschecking" attendance records and find discrepancies; do ya really think that they deduce quality of life is not happening for a particular resident???? I think we have to give surveyors more credit and trust their instincts and the baselines they are working off of, comparatively speaking. If you can verbally collaborate/discuss/provide reasons as to why individuals lack "proven" documentation via attendance, they will accept it. I honestly believe, they use insufficient documentation deficits to cite, when they know there are larger issues (under staffing in activities dept, burnt out director/staff, environmental deficits in program planning and old school approaches and content) The larger issues are so much more intangible and very difficult to quantify or "prove". Attendance record keeping, says virtually nothing about those issues as well as defining WHO a Resident is. Fundamentally, therapeutic recreation documentation practices( specific to Long Term Care) needs to be revamped/reformed. |

Thank you all for your positive comments! I really needed it! Other facilities Ive worked at gave us a cash bonus for being deficiency free. My administrator at this new facility did not even acknowlege my department even for 1/2 a second. She is so focused on the 2 deficiencies that nursing and dietary received. I realize I need to motivate and pat myself on the back but it gets exhausting as you all know. Thanks again for your positive comments! Patti |

Congratulations! Yes, we all work our b*tts off on documentation, revamping forms, trying to write or type clearly, on top of not sitting in the office actually doing that-and when we get praise from surveyors, but there are other deficiencies, we're part of the "answer" with extra daily/weekly checklists in those departments, more forms to fill out to show that everyone is checking up on the problem that was found, and there goes 20% of our time to continue our good work for Activities! But you DID do a great job! You WERE recognized positively by the people who can potentially make comments that could cost you your job! Your Residents respond well to you!! So hold your head high, and try not to allow yourself to feel like you have to walk on eggshells because of your successful program-you earned the praise!! |
